Work Environment

This position is primarily onsite, providing care directly within patients' homes. Team meetings and collaborative sessions also take place in our Mc Lean clinic, emphasizing strong communication and teamwork.

Key Responsibilities

• Conduct thorough evaluations by reviewing patients' medical histories, performing standardized assessments, and formulating tailored treatment plans based on individual needs.

• Engage with patients and their families to explain evaluation results, ensuring that they understand the treatment strategy and actively involve them in setting achievable goals.

• Implement evidence-based treatment protocols, adjusting techniques as necessary to accommodate patients' evolving health statuses and respond to changing needs in their home environments.

• Maintain close communication with fellow healthcare professionals to provide a multidisciplinary approach to patient care, recognizing and promptly addressing any changes in a patient's condition that may require adjustments to the care plan.

• Manage comprehensive discharge planning processes, ensuring appropriate follow-up care is in place. Keep meticulous documentation that reflects patient progress and supports optimal care transition.

• Utilize effective time management skills to meet productivity expectations while maintaining an accurate schedule. Participate in marketing initiatives to educate the community about available services.

• Contribute actively to departmental goals and comply with all relevant regulations, ensuring the clinic's operations adhere to the highest standards.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
