Key Responsibilities\n

• Collaboratively work with primary care practitioners to identify, treat, and manage mental health conditions and psychosocial factors, enhancing the overall effectiveness of the healthcare team.

• Conduct thorough patient intakes, focusing on diagnostic and functional assessments, and develop tailored therapy objectives that align with treatment plans.

• Deliver prompt intervention for clients requiring emergency mental health assistance through structured protocols.
\n
• Provide expert recommendations to primary care teams for patients whose complex needs may require specialized mental health services or additional referrals.

• Implement regular follow-up evaluations, assessing patient progress and efficacy of treatment, and adjusting plans as necessary to achieve optimal outcomes.

• Partner with clients to establish robust relapse prevention strategies and offer ongoing support for their mental health journeys.
\n
• Proactively identify patients at risk of worsening health conditions, contributing to the development of preventive care initiatives.
\n
• Oversee and coordinate the delivery of mental health services, ensuring integrated care and collaboration with involved external providers when needed.

• Maintain meticulous documentation of patient diagnoses, progress, and treatment plans to comply with Agency and Federal Qualified Health Center (FQHC) standards.

• Engage in consistent communication with primary care providers, keeping them apprised of developments in patient care and treatment plans.

• Facilitate behavioral health interventions that guide patients toward improved lifestyle choices, directly impacting their overall health.

• Equip patients with essential self-management skills and educational resources, empowering them to actively participate in their recovery.
\n
• Consult with clinic management and peers to optimize behavioral health services and explore avenues for program enhancement and evaluation.

• Support patients in adhering to any prescribed medical treatments initiated by their primary care practitioner, addressing any concerns related to medication.

• Uphold the highest standards of ethics as outlined by professional guidelines and complete additional duties as assigned.

Patient Population / Client Focus\n

This role will primarily serve clients from diverse backgrounds who are facing a myriad of mental health challenges. The demographic may include children and adolescents, adults grappling with anxiety, depression, trauma, and other behavioral health issues. As a telehealth counselor, you will tailor your approach to meet the unique needs of each patient, ensuring culturally competent care and establishing rapport across virtual platforms.

Qualifications\n

• A Master's Degree in Clinical Counseling or a related field from an accredited institution is required.

• A valid LPC (Licensed Professional Counselor) license is mandatory for this role.

• Adept understanding of mental health evaluation protocols, treatment planning methodologies, developmental psychopathology, and therapy modalities such as family therapy and group therapy.

• Fluency in both English and Spanish is highly valued, promoting wider communication with our patient base.

• Deep understanding of the social and psychological repercussions of mental illness on individuals and their families, along with community resource knowledge.

• Familiarity with psychodiagnostics and various psychotherapy concepts is advantageous.

Preferred Experience\n

Experience working within a telehealth model or similar remote service environments is preferred. Additionally, familiarity with collaborative treatment planning and previous work in multi-disciplinary health settings will be beneficial in this role.
