Work Environment

Your work environment will primarily consist of visiting patients in their homes, allowing you to create personalized rehabilitation plans tailored to their daily routines and specific needs. Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions, using a variety of assessment tools to develop individualized treatment plans.
• Implement evidence-based treatment interventions designed to enhance functional mobility and overall physical performance, continually monitoring patient progress and adjusting care plans as necessary.
• Maintain accurate and timely documentation of evaluations, treatment sessions, and patient outcomes in accordance with our organizational and regulatory standards.
• Engage with patients and their families to educate them about therapy processes, exercises, and any necessary adaptations to support rehabilitation and recovery.
• Collaborate with other healthcare professionals, including physicians and occupational therapists, to ensure comprehensive care and optimal patient outcomes.
• Attend regular team meetings and contribute to discussions on best practices, clinical innovations, and patient feedback.
• Participate actively in initiatives aimed at removing barriers to access, focusing on community support and engagement.

Patient Population / Client Focus

You will primarily work with patients recovering from orthopedic surgeries, sports injuries, and musculoskeletal conditions. This diverse patient population requires tailored treatment plans that address various functional limitations, empowering them to achieve their rehabilitation goals and regain independence. Your commitment to patient education and empathy will be essential in motivating and guiding them throughout their recovery journey.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited physical therapy program is required.
• A valid state license as a Physical Therapist (PT) from the Physical Therapy Examining Committee.
• A minimum of two years of clinical experience in outpatient orthopedic settings, demonstrating a strong understanding of orthopedic diagnoses and treatment protocols.
• Currently certified in CPR, ensuring readiness in emergency situations.

Preferred Experience

• Advanced experience in manual therapy techniques and functional training approaches.
• Familiarity with electronic medical records (EMR) systems and documentation software.
• Experience working in home health care settings or knowledge of home care protocols and regulations.

Compensation and Benefits
