This role is focused on delivering onsite care within a home care setting across the Matawan, NJ area. Key Responsibilities

• Conduct thorough evaluations of patients in their homes to assess their physical condition, functional abilities, and specific therapy needs.
• Develop individualized treatment plans that may include exercise regimens, manual therapy techniques, and the use of specialized equipment designed to alleviate pain and promote mobility.
• Educate patients and their families about treatment plans, expected outcomes, and self-management strategies, ensuring understanding and engagement in the rehabilitation process.
• Monitor patient progress closely, documenting all treatments and responses in an electronic health record system to provide clear communication and coordinate care among healthcare providers.
• Adapt treatment strategies based on ongoing assessments to meet the evolving needs of patients, including the recommendation and fitting of assistive devices, when necessary.
• Provide training and support to caregivers and family members on best practices for assisting patients during their rehabilitation journey.
• Stay abreast of the latest developments in physical therapy through participation in workshops, professional networks, and continuing education programs.

Patient Population / Client Focus

Your patients will include children and adults with a diverse array of conditions ranging from post-operative recovery, neurological disorders, to chronic pain management. This holistic approach requires a nuanced understanding of various diagnoses and the ability to tailor therapeutic strategies accordingly. Your expertise will help guide patients through their recovery processes, facilitating not only physical improvement but also enhancing their overall well-being.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited institution.
• Valid physical therapy license in New Jersey.
• Strong communication skills, capable of conveying complex information effectively to patients and healthcare teams.
• Proficient in word processing and familiar with electronic health records.
• Demonstrated flexibility and adaptability in a dynamic work environment.

Preferred Experience

• A minimum of 2 years of relevant physical therapy experience preferred, but new graduates are also encouraged to apply.
• Prior home health experience is a plus, enabling a smoother transition into the unique challenges of home care therapy.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 a year , commensurate with experience and qualifications. In addition, we offer a comprehensive benefits package that includes:
