Work Environment

This position is primarily onsite, and you will work in a home care environment, traveling to patients' residences, which provides a unique opportunity to create strong therapeutic relationships in a comfortable setting for your patients. Key Responsibilities

• Conduct comprehensive evaluations of patients, including thorough reviews of medical history, physical assessments, and establishing therapeutic goals in collaboration with patients and their families.
• Communicate evaluation results clearly and develop tailored treatment plans based on patient assessments, ensuring that interventions are designed to meet each individual's unique circumstances.
• Implement evidence-based treatment protocols, providing hands-on therapeutic interventions that facilitate recovery and promote independence in daily activities.
• Collaborate with interdisciplinary healthcare teams to provide holistic care, consulting other professionals as necessary to address any changes in the patient's condition or treatment requirements.
• Manage documentation rigorously, including detailed initial evaluations, daily progress notes, and discharge summaries, ensuring timely and accurate reporting of patient care activities.
• Exhibit effective time management skills while attending to client scheduling, meeting productivity metrics, and maintaining punctuality for patient visits and team meetings.
• Promote a patient-centered care experience by actively addressing any barriers to care access and continually engaging in customer service excellence.
• Participate in departmental objectives and initiatives, contributing to team discussions regarding quality improvement and compliance with relevant regulatory standards.
• Engage in continuing education and professional development activities, mentoring students and junior staff to cultivate a knowledgeable and skilled team.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy is required; a Doctorate degree (DPT) is preferred.

• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is required.
• A proven aptitude for working collaboratively within a healthcare environment, demonstrating both clinical and interpersonal skills.

Preferred Experience

• A minimum of 2 years of clinical experience as a Physical Therapist is preferred, with demonstrated skills in home care settings.

Compensation and Benefits
