Work Environment

The position is primarily onsite, focusing on home care, which allows for flexible engagement with patients in the comfort of their own environments. You will collaborate closely with a multidisciplinary team of healthcare professionals to develop and modify care plans that reflect the holistic needs of our patients. Key Responsibilities

• Conduct thorough patient evaluations, including the review of medical history, patient assessments, and diagnostic tests, to inform treatment decisions.
• Communicate assessment results and treatment plans to patients and their families, ensuring their involvement in goal setting to foster a sense of ownership over their recovery process.
• Demonstrate expertise in implementing evidence-based treatment protocols tailored to individual patient assessments and evolving healthcare standards.
• Collaborate with other healthcare professionals, recognizing when to adjust care plans in response to changes in patient conditions and referring to specialists as necessary.
• Execute comprehensive discharge planning and follow-up care, maintaining accurate and ethical documentation of patient care activities.
• Exhibit excellent time management by managing schedules effectively and meeting productivity goals, including attending mandatory meetings and engaging in marketing initiatives.
• Contribute to departmental goals and objectives while ensuring compliance with healthcare regulations and standards.
• Actively seek opportunities for professional development, including participation in continuing education courses and mentoring students as a clinical instructor.
• Enhance customer experiences by promoting flexibility and accessibility in care delivery during home visits.
• Engage in data collection and analysis when working on Access NRH initiatives to identify and remove barriers to care.

Patient Population / Client Focus

You will provide care to a diverse patient population, including seniors with age-related mobility issues, individuals recovering from surgeries or injuries, and patients with chronic conditions such as stroke or arthritis. Your role is pivotal in helping these patients regain independence and improve their quality of life, which can be life-changing.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited program.
• Current and valid Physical Therapy license in the State of New Jersey.
• American Heart Association's Basic Life Support (BLS) certification for Healthcare Providers.
• Open to new graduates; while prior work experience is preferred, it is not required.

Preferred Experience

• Prior experience in a home care setting is advantageous but not mandatory.
• Familiarity with community health resources and rehabilitation practice standards is a plus.
