Home Care Physical Therapist - Full-Time Position

Position Overview

Baylor Physical Therapy is seeking a dedicated and compassionate Home Care Physical Therapist to join our team in Keansburg, New Jersey. This full-time position allows you to make a significant impact on patients' lives as you provide rehabilitation services in their own homes. Work Environment

This role is primarily onsite, providing direct care in patients' homes. Our home care setting emphasizes flexibility and individualized treatment plans tailored to meet each patient's unique needs. As a member of our dedicated team, you will work collaboratively with other healthcare professionals, including Physical Therapy Assistants, case managers, and physicians, to ensure comprehensive care delivery.

Key Responsibilities

• Develop and implement individualized physical therapy treatment plans aimed at improving mobility, relieving pain, and restoring function.
• Oversee and guide Physical Therapy Assistants in executing treatment plans, ensuring compliance with best practices and patient safety.
• Evaluate patients' physical abilities using diagnostic tests and assessments to adjust the plan of care as needed.
• Assess the home environment for rehabilitation, providing recommendations that empower patients to regain independence.
• Administer therapeutic exercises, traction, massage, and other modalities to facilitate recovery.
• Monitor and document patient progress through clinical records, including evaluations, care plans, and discharge summaries.
• Cultivate strong communication channels with clients, families, and interdisciplinary team members to enhance patient outcomes.
• Educate clients and their families on the use and care of physical therapy devices and the overall treatment program.
• Participate actively in case conferences, contributing insights into the multi-disciplinary approach to patient care.
• Ensure timely submission of clinical documentation and other necessary reports per branch policy.

Patient Population / Client Focus

The Home Care Physical Therapist will primarily work with a diverse patient population, including elderly individuals, post-operative patients, and those managing chronic illnesses or disabilities. This role requires a deep understanding of various patient conditions and the ability to tailor interventions that promote physical, emotional, and social well-being. Your expertise will be instrumental in helping patients return to their daily activities and improve their overall quality of life.

Qualifications

• Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist in the State of New Jersey.
• Minimum of two years of relevant experience in a clinical setting, preferably in home care or acute care.
• Current CPR certification.
• Valid driver's license and automobile insurance.
• Strong interpersonal communication skills with a capacity to engage effectively with patients and families.

Preferred Experience

• Experience with electronic medical records (EMR) systems.
• Knowledge of federal and state regulations regarding home health care.

Compensation and Benefits

The Home Care Physical Therapist position offers a competitive salary range of USD 100,000 to 120,000 per year, depending on experience.
