Work Environment

This position offers the exciting opportunity to work onsite in a home care setting, allowing you to reach patients where they are most comfortable - in their own homes. The role requires flexibility and adaptability, as you will encounter diverse environments and patient conditions daily. Our clinic operates Monday through Friday with a full-time commitment of approximately 40 hours per week. Key Responsibilities

• Evaluate and conduct comprehensive assessments of patients to establish baseline functional abilities and therapeutic needs.
• Create, implement, and adjust individualized treatment plans tailored to each patient's specific goals, utilizing various therapeutic methods such as manual therapy, therapeutic exercises, and modalities.
• Utilize state-of-the-art equipment and therapeutic tools to enhance patient outcomes, ensuring you remain informed on the latest advancements in physical therapy technologies.
• Document treatment sessions meticulously, tracking patient responses and progress to refine ongoing care strategies.
• Regularly reassess therapeutic goals, proposing new plans and strategies as the patient progresses through their recovery.
• Provide education and training for patients, caregivers, and other health team members about rehabilitation protocols and the effective use of assistive devices.
• Commit to continuous professional development by participating in educational workshops, reviewing current literature, and engaging in professional networks.

Patient Population / Client Focus

The Home Care Physical Therapist will work with a diverse clientele, including elderly patients recovering from surgeries or managing chronic illnesses, as well as younger individuals rehabilitating from sports-related injuries. This role requires an empathetic and patient-centered approach, as therapy often takes place in a variety of home settings, which may include unique challenges. Building rapport with patients and fostering their independence will be key to successful outcomes.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program is required.
• A valid physical therapy license in the State of New Jersey.
• Exceptional communication skills, both written and verbal, to effectively interact with patients and healthcare teams.
• Proficient computer skills for documentation, scheduling, and managing patient information.
• High adaptability to handle varying patient needs and home environments.

Preferred Experience

While a minimum of two years of clinical experience is preferred, we welcome enthusiastic new graduates. Experience in home health care or geriatric physical therapy is a distinct advantage. Compensation and Benefits

This role offers a competitive salary ranging from USD 100,000 to USD 120,000 per year, complemented by a comprehensive benefits package.
