Work Environment

This position is located onsite at our facility, which delivers home healthcare services. Our clinic fosters a collaborative atmosphere where every team member's contributions are valued. Key Responsibilities

• Conduct thorough screening and evaluations of patients to assess physical ailments, utilizing both subjective and objective assessment methods.
• Develop individualized treatment plans based on accurate diagnosis, incorporating a variety of physical therapy modalities and procedures.
• Educate patients and their caregivers about treatment protocols, providing them with the necessary tools and knowledge to assist in the rehabilitation process.
• Document each patient's progress meticulously in compliance with regulatory standards and facility requirements, ensuring a robust continuum of care.
• Maintain department productivity standards by effectively managing your caseload and prioritizing tasks to ensure timely and quality service delivery.
• Engage in continuous education, staying updated on the latest evidence-based practices to enhance patient care and improve clinical outcomes.
• Participate actively in quality improvement initiatives, including safety audits and equipment protocols, guaranteeing optimal service delivery.
• Collaborate effectively with interdisciplinary teams, including nurses and physicians, to coordinate holistic patient care and address any emerging needs.
• Assist junior staff and physical therapy assistants in delivering high-quality care and ensure adherence to best practice standards.

The Physical Therapist will serve a wide range of patients from different demographics, including pediatric to geriatric populations, thus requiring a comprehensive understanding of varying conditions. Qualifications

• A current state Physical Therapist license is required.
• A Bachelor's degree in Physical Therapy from an accredited program is necessary; Master's or Doctorate in Physical Therapy is preferred.

Compensation and Benefits
