Key Responsibilities\n

• Coordinate seamless transitions for patients between hospital care and our community services, ensuring effective communication with hospital staff to facilitate discharges.
\n
• Utilize specialized crisis de-escalation techniques to provide immediate support to clients in distress, including taking on the role of Clinician of the Day as needed.
\n
• Conduct thorough risk assessments and develop comprehensive safety plans tailored to the individual needs of clients.
\n
• Document all client interactions and relevant information on the same day to maintain accurate and up-to-date records, particularly for those at heightened risk of crisis intervention.
\n
• File critical incident reports within 24 hours when necessary, adhering to organizational protocols for incident management.
\n
• Participate in an after-hours phone rotation to offer ongoing care coordination support outside standard hours.
\n
• Work proactively with clients to minimize the need for crisis services, fostering skills and resources for independent living.
\n
• Maintain crisis alerts for collaboration with county crisis services, ensuring proper coordination and support for clients requiring intervention.

The therapist will serve a diverse population within the community, including individuals of varying ages and backgrounds experiencing mental health challenges. Qualifications\n

• A Master’s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited program.

• Current, independent licensure in the state of Oregon (LCSW, LMFT, or LPC).

• Strong analytic and interpretive skills, exhibiting educated judgment and professional communication competency.

• Proven ability to remain calm in high-pressure situations, demonstrating empathy and effective listening skills.

• Willingness to prioritize and delegate tasks efficiently in a fast-paced environment.

Preferred Experience\n

• Experience working within community health settings or similar environments.

• Demonstrated ability to work effectively with high caseloads while maintaining high-quality service delivery.

Compensation and Benefits\n
