Work Environment

This is a full-time onsite position that primarily involves providing in-home care for patients. As a Physical Therapist in our organization, you will work directly in patients' residences, ensuring a comfortable and familiar environment conducive to rehabilitation. You will operate Monday through Friday from 9 AM to 5 PM, allowing for a structured yet flexible schedule that meets patients’ needs.

Key Responsibilities

• Conduct comprehensive physical assessments to identify movement disorders, balance issues, and mobility limitations, ensuring an accurate understanding of each patient's condition.
• Utilize clinical expertise to ascertain the root causes of pain and dysfunction through precise evaluations and evidence-based practices.
• Collaborate with patients to establish realistic rehabilitation goals and consistently monitor their progress through regular assessments and modifications to their treatment plans.
• Develop and implement individualized rehabilitation programs that address the specific needs and goals of each patient, adapting strategies as necessary based on their feedback and progress.
• Maintain thorough and timely documentation of patient interactions and therapeutic activities, ensuring compliance with regulatory standards and best practices in record-keeping.
• Educate patients and their families on prevention strategies, effective rehabilitation exercises, and holistic wellness approaches to empower them in their recovery journey.
• Stay informed on industry best practices, emerging trends, and advancements in physical therapy through ongoing education and professional development opportunities.

Patient Population / Client Focus

As a Physical Therapist at InCore, you will work primarily with diverse patients in home care settings, including individuals recovering from orthopedic surgeries, those with chronic pain conditions, and patients seeking rehabilitation from sports-related injuries. Your compassionate, patient-centered approach will play a vital role in instilling confidence and motivation in patients as they navigate their recovery processes.

Qualifications

• Valid Physical Therapist (PT) license in the state of New Jersey.
• Bachelor's Degree in Physical Therapy or higher (such as a Doctor of Physical Therapy - DPT).
• Proven ability to develop and execute patient-centered treatment plans that are effective and measurable.
• Excellent communication and interpersonal skills for effective patient interactions and teamwork.
• Strong analytical skills with the capacity to evaluate patient conditions and modify plans accordingly.
• A compassionate demeanor and a commitment to delivering exceptional care.
• Ability to work independently within patients' homes as well as collaboratively with an interdisciplinary team.

Preferred Experience

• A minimum of two years of relevant clinical experience in physical therapy.
• Experience in home care settings or outpatient rehabilitation.

Compensation and Benefits

We offer a competitive annual salary ranging from $100,000 to $120,000, commensurate with experience.
