This position is primarily onsite, requiring you to travel to our clients' homes across Clearwater and the surrounding areas. You will work a structured schedule from Monday to Friday, enabling you to balance work and personal life effectively. The Home Care Physical Therapist plays a vital role in the assessment and treatment of clients using evidence-based practices. Formulating and directing individualized physical therapy treatment plans aimed at preventing disability, relieving pain, and restoring muscle function based on each client's unique needs.

Overseeing physical therapy assistants to ensure they execute treatment plans effectively and safely.

Conducting comprehensive evaluations of clients' functional abilities and home environments, tailoring rehabilitation strategies to enhance their independence.

Implementing hands-on therapy through manual exercises, administering traction, and utilizing physical agents for treatment.

Adjusting and fitting orthotic and prosthetic devices in collaboration with other healthcare professionals as necessary.

Maintaining meticulous clinical records, including initial evaluations, progress notes, and discharge summaries, to track patient progress and aid in ongoing care adjustments.

Participating actively in interdisciplinary team meetings, providing insights on treatment options and contributing to holistic care planning.

Educating clients and their families on physical therapy methodologies, home exercise programs, and the care of therapeutic devices.

Submitting timely documentation to comply with organizational policies and ensure comprehensive care delivery.

Staying accessible to physical therapy assistants to provide guidance and support during client treatments, ensuring the highest standard of care.

Our clients range from individuals recovering from surgery or injury to patients managing chronic conditions and those seeking preventative care. You will work closely with diverse populations, including the elderly, individuals with disabilities, and post-operative patients. Understanding their specific challenges and goals is crucial to delivering effective care and achieving positive outcomes.

A Bachelor's degree in Physical Therapy from an accredited program.

A minimum of one year of experience in a physical therapy role, preferably within home care or an acute care setting.

Current state licensure as a Physical Therapist in Florida, in good standing.

Current CPR certification to ensure readiness in emergency situations.

A valid driver's license and automobile insurance for travel to client locations.

While the minimum experience required is one year, candidates with additional experience in home healthcare settings, particularly those working with geriatric or disabled populations, will be given preference. Additionally, a proven ability to work effectively as part of an interdisciplinary team will be a strong asset.
