Physical Therapist - Full Time - Home Health Care

Position Overview One Tap Physio Care is seeking dedicated and skilled Physical Therapists to join our outpatient clinic team in Port Charlotte, Florida. As a Full-Time Outpatient Physical Therapist, you will play a crucial role in providing personalized evaluation and treatment services, paving the way for improved patient mobility and enhanced quality of life. This position will have you working onsite in a home care setting, providing therapy for patients in the comfort of their own homes. You will collaborate closely with a team of healthcare professionals and actively engage with patients' family members, facilitating a supportive environment that promotes recovery and healing. Our team operates under a Monday to Friday schedule, allowing for a healthy work-life balance while delivering exceptional care to our community.

Document patient progress meticulously in compliance with state regulations and organizational policies, ensuring that all data reflects the patient journey and treatment efficacy.

Proactively educate patients and their families, equipping them with necessary information on exercises, rehabilitation techniques, and self-care practices to enhance recovery.

Collaborate with interdisciplinary teams to coordinate care plans and ensure comprehensive treatment approaches.

Our clinic serves a diverse patient population, including individuals recovering from surgeries, injuries, neurological conditions, and chronic pain syndromes. By understanding the unique challenges faced by each patient, our therapists work to provide individualized care that not only addresses immediate concerns but also fosters long-term wellness and resilience.

Bachelor's Degree in Physical Therapy from an accredited institution.

While we welcome newly graduated professionals, candidates with prior experience in outpatient settings and those skilled in advanced therapeutic modalities will be preferred. Experience with geriatric populations or post-surgical rehabilitation will also be considered an asset.
