This position is primarily remote, allowing flexibility in your work setting while maintaining a focus on delivering quality mental health services through telehealth. As an LPC, you will collaborate with a multidisciplinary team of healthcare professionals, including primary care providers, to ensure a holistic approach to each patient’s treatment. Key Responsibilities\n

• Support primary care providers by collaborating on the recognition, treatment, and ongoing management of mental health and psychosocial issues, contributing actively to the interdisciplinary treatment team.

• Conduct thorough patient intakes, focusing on diagnostic assessments and functional evaluations, providing constructive feedback to enhance the patient's treatment plan.

• Deliver brief, targeted interventions for clients in need of mental health services, ensuring that interventions align with established clinical guidelines.

• Advise primary care providers on whether patients require specialized mental health services or can be effectively managed within the primary care setting.

• Monitor patient progress through follow-up assessments and adjust treatment strategies as necessary, ensuring continuity of care.

• Develop relapse prevention plans tailored to individual patient needs, aimed at helping clients maintain stability and prevent recurrence of issues.

• Identify at-risk patients and implement proactive strategies to mitigate deterioration in mental health status.
\n
• Coordinate the delivery of health services related to behavioral health, establishing links with other treatment providers while respecting patient confidentiality.

• Document patient progress comprehensively in treatment charts, ensuring all information is up-to-date and reflective of the patient’s journey.

• Maintain open communication with primary care providers about patient needs and progress, collaborating on treatment plans to enhance outcomes.

• Support patients in making informed lifestyle choices and implementing self-management skills, empowering them to actively participate in their treatment and recovery.

• Provide consultation and insights regarding behavioral health services to clinic management and team members, contributing to program evaluation and outcomes improvement.

• Assist patients in aligning with any medical treatments recommended by primary care, including coping strategies for medication side effects.

• Adhere to the Code of Ethical Conduct for Clinical Professionals in all interactions.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• Active LPC (Licensed Professional Counselor) License.

• Ability to utilize DSM-IV diagnostic criteria in practice.

• Bilingual proficiency in English and Spanish is preferred but not mandatory.

Compensation and Benefits\n
