Work Environment

This position is based in Santa Clara, CA, where you will work onsite in the community, directly in patients' homes. This unique setting allows for a personalized approach to therapy, enabling you to create customized treatment plans that meet the specific needs of each patient in their own environment. Your typical work hours will be from Monday to Friday, between 9:00 AM and 5:00 PM, providing you with a stable work-life balance.

Key Responsibilities

• Conduct comprehensive patient assessments to determine physical therapy needs and develop individualized treatment plans based on evidence-based methodologies.
• Deliver one-on-one patient care, typically seeing 1-2 patients per hour, and adjust treatment plans as necessary based on patient progress and feedback.
• Utilize a variety of therapeutic techniques, including manual therapy, exercise prescription, and modalities to facilitate optimal recovery.
• Document all patient interactions and treatment plans using our Electronic Medical Records (EMR) system, ensuring accurate and up-to-date records.
• Collaborate with a multidisciplinary team of healthcare professionals to coordinate comprehensive care and optimize treatment outcomes.
• Educate patients and their families on home exercise regimens and self-care techniques to promote long-term health and wellness.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic surgeries, injuries, and chronic conditions that affect mobility and quality of life. This often includes elderly patients, post-operative rehabilitation cases, and individuals seeking to regain functional independence. Being able to adapt your therapeutic techniques to cater to diverse patient needs will be essential for success in this role.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field from an accredited institution.
• A valid California Physical Therapist license or eligibility to obtain licensure.
• Strong analytical and problem-solving skills to tailor treatments based on patient assessments.
• Excellent interpersonal and communication abilities to engage effectively with patients and their families.

Preferred Experience

• A minimum of two years of professional experience in orthopedic physical therapy, preferably in an outpatient clinic.
• Previous experience working with home care patients is strongly desired.

Compensation and Benefits

This role offers a competitive salary ranging from USD 120,000 to 130,000 annually, commensurate with your experience and background.
