Work Environment

You will primarily work in a home care setting, allowing for a personalized therapeutic environment tailored to each patient's unique needs. This onsite role involves traveling to patients' homes within Sunnyvale, where you will collaborate closely with families and caregivers to develop and implement effective rehabilitation plans.

Key Responsibilities

• Conduct initial evaluations and ongoing assessments of patient progress, providing one-on-one therapy sessions that cater to individual needs.
• Design and implement customized rehabilitation programs utilizing the latest evidence-based techniques, with a focus on improving mobility and reducing pain.
• Document patient progress in detail using WebPT, ensuring compliance with healthcare regulations and enabling seamless collaboration with multidisciplinary teams.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• An active California Physical Therapy license or eligibility for licensure is essential.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 120,000 to 130,000, commensurate with experience, alongside a comprehensive benefits package. This includes medical, dental, and vision insurance, as well as a 401(k) plan with employer contributions.
