Senior Physical Therapist - Onsite Home Care Specialist

Position Overview

Functional Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our innovative team in San Jose, California. This full-time position will focus on providing exceptional rehabilitation services in an onsite home care setting. Work Environment

This position is based entirely onsite in the homes of our patients, allowing for a unique and personalized approach to therapy. You will work Monday through Friday from 9:00 AM to 5:00 PM, providing a consistent schedule that promotes work-life balance. As part of a small, dedicated team, you will collaborate closely with colleagues to share insights and strategies that enhance patient care.

Key Responsibilities

• Provide direct patient care, working with 1 to 2 patients per hour to deliver focused, individualized treatment.
• Develop and implement customized rehabilitation plans utilizing evidence-based techniques to effectively improve patient mobility and reduce pain.
• Continuously monitor patient progress and adjust treatment plans based on individual responses and recovery timelines.
• Maintain meticulous patient records utilizing WebPT, ensuring accurate tracking of treatment progress and communication with the multidisciplinary care team.
• Educate patients and their families about treatment plans, exercise programs, and lifestyle modifications to facilitate optimal recovery.
• Actively participate in team meetings to discuss patient care strategies and outcomes, fostering a collaborative environment for continuous improvement.

Patient Population / Client Focus

Our patient population primarily includes individuals recovering from orthopedic surgeries, injuries, and chronic pain conditions. The scope of care focuses on pediatric and adult patients, offering a diverse range of rehabilitation challenges. As a Physical Therapist at Functional Physical Therapy, you will deeply engage with each patient, adapting your approach based on their specific needs and recovery aspirations.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required; candidates with a Master's or Doctorate degree are preferred.
• Possession of an active California Physical Therapy license or the ability to secure one before commencing work.
• Strong clinical reasoning skills, complemented by hands-on physical therapy experience in an outpatient or home care setting.
• Exceptional communication skills with the ability to motivate and engage patients effectively throughout their rehabilitation journey.

Preferred Experience

A minimum of 2 years of clinical experience in physical therapy is required. Familiarity with WebPT or other electronic medical record systems is a plus, along with a solid understanding of contemporary rehabilitation methodologies and patient care best practices.

Compensation and Benefits
