Key Responsibilities\n

• Develop, coordinate, and implement customized support plans tailored to help clients effectively address their personal challenges and overcome dependencies.

• Conduct thorough interviews with patients to evaluate their emotional and psychological condition, determining their strengths, weaknesses, and primary needs.

• Address each case with a holistic perspective, establishing specific, measurable, attainable, relevant, and time-bound (SMART) goals for progress evaluation.

• Continuously monitor and assess clients’ progress and modify treatment plans as necessary to ensure optimal outcomes.

• Provide guidance and counseling during sessions, offering informed recommendations on future actions and coping strategies.
\n
• Maintain accurate and detailed case records, ensuring that all documentation reflects clients’ statuses and treatment pathways.

• Participate in case conferences and provide expert testimony in court when required, advocating for the best interests of the clients.

• Act as a key reference point for clients within the multidisciplinary team, working collaboratively to ensure coordinated care.

• Facilitate referrals to pertinent community services that can support clients’ recovery and reintegration into their daily lives.

Patient Population / Client Focus\n

In this role, you will predominantly interact with clients at the hospice center, including individuals facing terminal illnesses and their families. Your focus will be on providing emotional support and therapeutic interventions to help them navigate complex feelings of grief, anxiety, and uncertainty. Qualifications\n

• A Master's degree in Social Work (MSW), psychology, or sociology is required, along with a current state licensure as a Clinical Social Worker (LCSW).

Compensation and Benefits\n
