Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate the mental health status of patients in hospice care, identifying their strengths, needs, and preferences.

• Utilize a range of evidence-based therapies, such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), to provide individualized treatment to patients and their families.

• Collaborate closely with a range of healthcare professionals, including psychiatrists and nurses, to ensure a cohesive and comprehensive approach to patient care.

• Educate patients and their families about diagnoses, treatment plans, and medication management to empower them in their care processes.

• Facilitate and lead individual, group, and family therapy sessions while utilizing evidence-based modalities tailored to the specific needs of patients.

• Develop and implement treatment plans in partnership with patients, addressing their goals and incorporating feedback from the entire treatment team.

• Advocate for patients within the complex mental health system, helping them to navigate resources and services that align with their needs.

• Organize and facilitate therapeutic groups and psychoeducation sessions, as well as contribute to discharge planning meetings to ensure continuity of care.

Patient Population / Client Focus\n

In this role, you will primarily work with adult patients in hospice care who are experiencing significant emotional and mental health challenges as they approach the end of life. Your expertise will provide crucial support to both patients and their families, helping them cope with grief, anxiety, and preparation for end-of-life transitions.

Qualifications\n

• A Master's degree in Social Work from an accredited program is required.

• Active Licensed Clinical Social Worker (LCSW) licensure in the state of California.

Compensation and Benefits\n
