Key Responsibilities\n

• Conduct comprehensive mental health assessments that take into account patients' medical histories and emotional challenges, ensuring holistic understanding of their needs.

• Design and implement personalized treatment plans that integrate strategies for both mental and physical health, considering the unique circumstances of each patient.

• Facilitate both individual and group therapy sessions, employing evidence-based methodologies tailored to the specific needs of your patients to encourage their personal growth and recovery.

• Work closely with medical providers to promote a coordinated care approach, facilitating communication regarding treatment plans and progress to ensure comprehensive patient support.

• Provide timely crisis intervention by assessing and responding to mental health emergencies, ensuring patient safety and effective resolution of urgent issues.

• Educate patients about the connections between mental health and physical wellness, empowering them to take an active role in managing their overall health.

• Maintain thorough and confidential documentation in compliance with legal and ethical standards, ensuring accuracy and privacy in patient records.

• Participate in regular team meetings, case conferences, and interdisciplinary care planning to enhance collaborative approaches in patient care.

• Stay current with the latest research and best practices in mental health treatment and integrated care, implementing new techniques to enhance our services.

Patient Population / Client Focus\n

You will primarily work with adults dealing with mental health issues exacerbated by co-occurring medical conditions. Your role will include supporting individuals through a variety of challenges, including anxiety, depression, trauma, and chronic illnesses. The focus will be on promoting recovery and enabling patients to navigate their paths to wellness, thereby enhancing their overall quality of life.

Qualifications\n

• A Master's or Doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current and valid licensure as a Licensed Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York State.

• A minimum of two years of experience providing mental health counseling services, particularly with patients facing co-occurring medical conditions.

• Strong interpersonal and communication skills, demonstrating compassion and understanding for patients' experiences.
\n
• Ability to maintain professional boundaries and uphold ethical guidelines while ensuring the confidentiality of all patient interactions.

• Proficiency in using electronic health record systems and other relevant technology to manage patient information effectively.

Compensation and Benefits\n

The salary for this position ranges from USD 80,000 to 100,000 annually, commensurate with experience.
