Key Responsibilities\n

• Conduct comprehensive assessments to understand and address the unique needs of each patient.
\n
• Facilitate open communication among supervisors, direct care staff, family members, and key stakeholders regarding patient progress and care plans.
\n
• Develop and provide a tentative schedule to supervisors, detailing appointments, home visits, and necessary observations for tracking patient progress.
\n
• Coordinate in-home visits, including observations and chart reviews, ensuring data integrity and comprehensive patient analysis.
\n
• Ensure implementation of individualized program plans is effective and meets established goals.
\n
• Participate actively in Interdisciplinary Team Meetings and other necessary discussions to ensure cohesive patient care.
\n
• Prepare and document monthly and quarterly progress notes, clearly articulating patient developments and any changes in care plans.
\n
• Organize training sessions to certify staff on the effective implementation of program protocols and documentation practices.
\n
• Conduct follow-up interviews and assessments to continuously support the evolving needs of patients.
\n
• Maintain comprehensive records, ensuring that relevant information is accessible to authorized personnel.
\n
• Act as a liaison between the practice and the broader mental health community to support and further develop care goals.
\n
• Assist in creating and refining training materials aimed at improving therapeutic programs.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in Pennsylvania.

• A minimum of 2 years of experience in counseling, preferably within a clinical setting.

• Strong understanding of legal and ethical standards in mental health practice.

• Exceptional interpersonal, communication, and rapport-building skills.
\n
• Demonstrated ability to work collaboratively within a healthcare team.

• Proficiency with Electronic Medical Records (EMR) systems and relevant technology.
