Key Responsibilities\n

• Collaborate with multidisciplinary teams, including physicians, nurses, and caregivers, to monitor patient outcomes and adjust care strategies as necessary.

• Provide direct support to families, delivering information, resources, and emotional guidance to help them navigate hospice services.

• Maintain compliance with all applicable federal and state regulations governing hospice and palliative care.

• Utilize evidence-based practices and the Natural Learning Environment model to foster a nurturing atmosphere for both patients and families.

• Develop and recommend joint treatment plans, while also evaluating service frequency based on individual patient progress and family dynamics.

• Deliver ongoing education to families about enhancing their loved one's quality of life during hospice care.

• Ensure the wellbeing of both staff and clients by adhering to all health and safety standards.

Patient Population / Client Focus\n

The LCSW will primarily engage with patients and families in a hospice setting, focusing on individuals experiencing life-limiting illnesses. This includes working with diverse populations, understanding their unique cultural, emotional, and psychological needs.
