Key Responsibilities\n

As a Licensed Professional Counselor, you will:

• Conduct thorough patient intakes focusing on diagnostic and functional assessments, providing well-informed recommendations regarding treatment goals and plans.

• Provide brief, targeted interventions for patients in need of mental health services, considering their unique circumstances and treatment needs.

• Engage in ongoing follow-up with patients to monitor progress, adapting treatment approaches as necessary.

• Develop relapse prevention plans where appropriate, helping patients maintain stability and prevent deterioration of their condition.

• Identify at-risk patients and collaborate with the care team to create preventive strategies tailored to individual needs.

• Coordinate and monitor the delivery of mental health services, connecting patients with relevant treatment providers both within and outside the clinic.

• Document patient progress and maintain accurate treatment records, ensuring comprehensive care continuity.

• Work collaboratively with primary care providers to formulate effective treatment plans and remain informed about patient needs and developments.

• Equip patients with self-management skills and educational resources to empower them in their recovery journey.

• Provide consultations to clinic management and team members concerning behavioral health services, suggesting timely evaluations to assess program effectiveness.

• Support patients in adhering to medical treatments as prescribed by primary care providers, including strategies for managing medication side effects.

• Uphold the Code of Ethical Conduct for Clinical Professionals and perform other duties as assigned.
