Key Responsibilities\n

• Conduct thorough and ongoing assessments to determine patient needs and develop tailored treatment plans.

• Prepare a tentative schedule for appointments, home visits, and monitoring activities, facilitating efficient tracking of each client’s progress.

• Coordinate and conduct home visits in line with therapy objectives, ensuring comprehensive understanding through observations and data review.

• Guarantee that the implementation of treatment plans aligns with organizational standards and effectively supports patient goals.

• Engage actively in Interdisciplinary Team Meetings, contributing insights and updates on patient needs and group objectives.

• Document monthly and quarterly progress notes to track patient developments, ensuring clarity for future reference and accountability.

• Lead training sessions to certify team members in the effective implementation of treatment plans and relevant documentation procedures.

• Conduct follow-up interviews and observations to ensure continued support for individuals facing ongoing challenges.

• Act as a liaison with community resources and mental health organizations to facilitate networking and support in achieving developmental goals.

Qualifications\n
