Key Responsibilities

Your day-to-day responsibilities will include:

• Conducting thorough evaluations of patients, including assessing their medical history, performing specific assessments, and determining necessary treatment recommendations.

• Effectively communicating evaluation findings and treatment plans to patients and their families, ensuring their involvement in goal-setting to empower their recovery journey.

• Implementing individualized treatment protocols based on comprehensive evaluations, patient presentations, and current healthcare practices.

• Collaborating with other healthcare professionals as needed, documenting any changes in patient condition that require alterations to the care plan, and consulting for additional medical or rehabilitation support when necessary.

• Managing discharge planning and follow-up care by providing clear and detailed documentation of the patient's treatment process, including evaluations, daily notes, and progress summaries.

• Maintaining effective time management to meet productivity standards, being punctual for appointments and meetings, and contributing to marketing strategies that enhance community engagement.

• Contributing to departmental goals and objectives while ensuring compliance with all governmental and accreditation regulations.

• Promoting an excellent customer experience by demonstrating flexibility and adaptability in patient interactions and care processes.

• Actively participating in initiatives aimed at improving access to care, understanding the metrics that inform service quality and engaging in strategies to enhance patient accessibility.

Patient Population / Client Focus

The patients you will serve at Gentle Motion Physical Therapy encompass a diverse range of conditions, from those recovering from surgery to individuals with chronic pain. Your role will involve assessing each patient holistically and developing tailored treatment plans that not only address their physical needs but also support their emotional and psychological well-being, thereby fostering a comprehensive recovery journey.

Qualifications

• A bachelor's degree in Physical Therapy from an accredited institution is required.

• Current physical therapist license as required by state regulations.

• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association is mandatory.

Preferred Experience

The ideal candidate will possess at least two years of professional experience in a physical therapy setting, ideally within outpatient rehabilitation. Previous experience managing a diverse patient population and familiarity with electronic health record (EHR) systems will be beneficial for this role.
