Key Responsibilities\n

• Collaborate with primary care providers to recognize, treat, and manage mental health and psychosocial issues, contributing to the overall well-being of patients.

• Conduct thorough patient intakes, focusing on diagnostic evaluations and functional assessments, and recommend treatment plans to primary care providers based on findings.

• Utilize evidence-based therapeutic interventions to provide brief and focused support to patients dealing with a variety of mental health concerns.

• Evaluate patient needs to determine whether referral to specialty mental health facilities is necessary, ensuring patients receive the appropriate level of care.

• Implement follow-up protocols to monitor patient progress and adjust treatment strategies as needed to achieve optimal outcomes.
\n
• Create relapse prevention plans when indicated, assisting patients in maintaining stable functioning and preventing exacerbation of conditions.
\n
• Identify at-risk patients and develop proactive strategies to mitigate further decline in their mental health.
\n
• Coordinate with other treatment providers, ensuring comprehensive management of behavioral health services.
\n
• Document patient details, treatments, and progress in clinical records, upholding the standards of care required by our agency and FQHC guidelines.
\n
• Work in concert with primary care providers to craft treatment plans responsive to the unique needs of individual patients.
\n
• Equip patients with self-management skills and educational resources necessary for self-directed participation in their treatment process.
\n
• Provide consultation to clinic management and team members regarding behavioral health services and areas for program evaluation.
\n
• Assist patients in navigating medical treatments initiated by their primary care provider, especially concerning coping strategies for medication side effects.
\n
• Adhere strictly to the Code of Ethical Conduct for Clinical Professionals while performing all duties.
\n
• Take on additional responsibilities as assigned by the management.

Patient Population / Client Focus\n

The patients you will work with are diverse, spanning various age groups and backgrounds with a wide range of mental health issues. Many of our clients present with chronic stress, anxiety, depression, and other psychological challenges. Your role will include establishing trusting relationships with them, and using your expertise to support their journey toward recovery and improved mental health.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.

• Candidates must hold a current LPC (Licensed Professional Counselor) license.
\n
• Extensive knowledge of mental health assessment techniques, treatment planning, and psychotherapeutic theories is essential.

• A strong understanding of developmental psychopathology, family and group therapy methods, and DSM-IV diagnoses is expected.
\n
• Excellent written and verbal communication skills, with an ability to express ideas effectively.

• Ability to assess individual patient needs and evaluate care effectiveness.
\n
• A healthy mental and physical state sufficient to meet the demands of the role.
\n
• Ability to establish strong working relationships with patients, families, and other professionals is crucial.

Preferred Experience\n

Compensation and Benefits\n

The salary range for this licensed position is competitive, between USD 87,000 and 105,000 annually. In addition to a robust salary package, we offer a sign-on bonus for new hires.
