Key Responsibilities\n

• Conduct one-on-one client meetings to assess personal goals, preferences, and available therapeutic options before developing tailored rehabilitation plans.

• Create and implement comprehensive rehabilitation plans that align with each client's education level, aptitudes, and future employment objectives, ensuring adjustments are made as necessary throughout the process.

• Collaborate closely with a diverse team of healthcare providers, including physicians, occupational therapists, and psychologists, to design and execute rehabilitation programs tailored to the unique needs of each client.

• Maintain accurate and up-to-date client case files and records that encompass personal data, eligibility, insurance details, services provided, and relevant correspondence.

• Systematically monitor client progress to assess goal achievement, making necessary modifications to ensure ongoing improvement and success.

• Establish consistent communication with clients participating in job training, occupational placement programs, and other initiatives, addressing any challenges that may emerge.

• Engage with clients' families and support networks to identify effective strategies to meet client needs.

• Timely analyze client needs and document comprehensive reports to facilitate seamless care coordination.

• Provide recommendations and referrals to community resources to enhance client support.

• Clinical Experience: Previous experience in mental health treatment settings, particularly with clients experiencing mood disorders, is essential.
