Key Responsibilities\n

• Develop and implement personalized support plans to assist clients and their families in coping with life-limiting illnesses, including grief and loss issues.

• Conduct comprehensive assessments of clients’ psychosocial needs, strengths, and challenges to inform individualized care strategies.

• Establish measurable goals tailored to each client's unique situation, allowing for effective tracking and modification of treatment plans.

• Regularly monitor client progress and adapt interventions to ensure maximum efficacy in addressing evolving needs.

• Provide evidence-based counseling and psychoeducation during individual and family sessions, guiding clients in making informed decisions regarding their care.

• Maintain thorough and accurate documentation of client interactions, treatment interventions, and progress assessments in compliance with regulatory standards.

• Collaborate with other social workers, mental health professionals, and medical staff in case conferences to provide holistic support.

Patient Population / Client Focus\n

This position will focus on clients within a hospice care setting, where emotional and psychological support is critical. Qualifications\n

• Master’s Degree in Social Work (MSW) with a valid state license as a Licensed Clinical Social Worker (LCSW).
