Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify clients' mental health needs, challenges, and strengths.

• Develop and implement individualized treatment plans through evidence-based therapeutic modalities, regularly reviewing and adjusting care according to client progress.

• Collaborate with multidisciplinary teams, including family members, healthcare providers, and community resources, to coordinate comprehensive care and facilitate smooth transitions between different levels of service.

• Diagnose and treat a range of mental health conditions, such as anxiety, depression, trauma, and behavioral disorders, adapting treatment plans as required.

• Facilitate referrals to supplemental services, such as specialized treatment programs, support groups, and psychiatric consultations, ensuring clients have access to all necessary resources.

• Comply with all relevant federal, state, and local regulations, acting in accordance with the Mental Health Practice Act and the ethical standards of the profession.

• Provide social work consultations and crisis intervention as needed, supporting clients and their families during times of emotional turmoil and bereavement.

• Effectively communicate with various teams and agencies to optimize patient care and address any delays or issues that may arise during discharge planning.

• Advocate for clients to ensure their voices are heard in care planning discussions, fostering an environment of support and empathy.

• Identify and address complex discharge needs, coordinating with care teams to minimize hospital readmissions and enhance client outcomes.

Qualifications\n

• Master's Degree in Social Work, Counseling, Psychology, or a closely related field from an accredited institution.
