Key Responsibilities\n

• Communicate regularly with physicians and multidisciplinary team members to discuss client progress, needs, and potential concerns, ensuring that care is coordinated and comprehensive.

• Maintain accurate and up-to-date clinical records, including progress notes and treatment plans, in compliance with department policies and state regulations.

• Work collaboratively with the healthcare team to develop individualized treatment plans based on psychosocial assessments, utilizing evidence-based practices to guide interventions.

• Provide crisis intervention and counseling for clients experiencing emotional or behavioral issues, developing strategies that are both immediate and sustainable.
\n
• Assist clients with discharge planning to ensure a smooth transition out of therapy, coordinating with various service providers within the community to assist in accessing further support.

• Engage and consult with clients' families or support systems throughout the treatment process, enhancing the support network surrounding the client.

Qualifications\n

• A minimum of three years of experience in an outpatient mental health setting, demonstrating the ability to deliver effective psychotherapy and treatment.
