Work Environment\n

This position is primarily remote, allowing for a flexible work-life balance while you engage with our patients. You will collaborate closely with a multidisciplinary team that includes nurses, medical doctors, and administrative staff within a Federally Qualified Health Center (FQHC) model, ensuring that our patients receive coordinated and comprehensive care. While the position offers remote options, occasional on-site meetings or training may be required in our Pasadena office.

Key Responsibilities\n

• Conduct thorough patient intakes, focusing on diagnostic evaluations and functional assessments, and collaborate with primary care providers to establish effective treatment goals.

• Deliver focused, brief therapeutic interventions for a diverse patient population, adapting techniques according to individual needs.

• Guide primary care providers in identifying which patients would benefit from specialty mental health services versus those who can be maintained within the primary care environment.

• Initiate follow-up communications with patients to monitor progress, provide encouragement, and adjust treatment plans as necessary.

• Develop individualized relapse prevention plans to support ongoing patient stability and recovery.
\n
• Identify “at-risk” patients early and create tailored strategies to mitigate the deterioration of their mental health.
\n
• Coordinate behavioral health services, consolidating care pathways with external specialists as permitted by patients.
\n
• Document all patient interactions, progress notes, and treatment plans accurately within electronic health record systems to ensure compliance with FQHC guidelines.
\n
• Advise on behavioral health program effectiveness and offer insights for quality improvement initiatives to clinic management.
\n
• Support patients in understanding and adhering to medical treatment instructions from primary care providers, including managing medication side effects.

Qualifications\n

Compensation and Benefits\n

This full-time position offers a competitive annual salary range between USD 87,000 and 105,000, commensurate with experience and qualifications.
