Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews and observations, along with a thorough review of medical records, to evaluate the various emotional, social, and environmental factors affecting each patient's recovery journey.

• Provide empathetic counseling and support, helping patients and their families navigate complex emotional challenges related to their health conditions and life adjustments.

• Work closely with a multidisciplinary rehabilitation team to devise individualized treatment plans that integrate psychosocial considerations into each patient’s care.

• Facilitate access to community resources, including support groups and vocational rehabilitation programs, ensuring patients have the necessary tools and support post-discharge.

• Deliver educational sessions to both patients and their families about available community resources and essential post-rehabilitation services.

• Contribute to discharge planning efforts, assisting in smooth transitions for patients moving back into their homes or community settings.

Patient Population / Client Focus\n

Our patients include individuals of all ages who are recovering from various conditions such as stroke, spinal cord injuries, or orthopedic surgeries. By addressing their emotional and social needs, the LCSW plays a vital role in the rehabilitation process, ensuring that patients feel supported throughout their recovery and focused on their return to full independence.

Qualifications\n

• A Bachelor’s Degree in Social Work is mandatory; a Master’s degree is preferred.

• A current and valid LCSW license is required to practice in the state of Colorado.
