Key Responsibilities\n

• Conduct comprehensive assessments to determine the specific needs and psychological frameworks of patients, tailoring interventions accordingly.

• Create and maintain a schedule of appointments, home visits, and observations to ensure continuity of care and effective monitoring of patient progress.

• Coordinate home visits aligned with care requirements, enabling more personalized observations, chart reviews, and interviews that inform treatment strategies.

• Implement and review treatment plans, ensuring they are executed effectively and adjusted based on ongoing evaluations.

• Engage in interdisciplinary team meetings and relevant consultations to share insights, recommend best practices, and collaborate on patient care strategies.

• Document monthly and quarterly progress notes that capture patient developments, contributing to a comprehensive understanding of treatment efficacy.

• Lead training sessions for staff and other team members, ensuring competency in program implementation and documentation practices.

Qualifications\n
