Key Responsibilities\n

• Conduct comprehensive assessments and evaluations to understand the needs of each patient, ensuring a patient-centered approach to treatment.

• Engage in regular communication with supervisors, direct care staff, family members, and other stakeholders to provide updates on patient progress and treatment plans.

• Develop and present tentative schedules for appointments and home visits, strategically planning to monitor and support client progress.
\n
• Coordinate and execute home visits that may include patient observations, data analysis, and chart reviews, ensuring that the information gathered is utilized to enhance therapy outcomes.

• Participate actively in Interdisciplinary Team Meetings, contributing insights and collaborating with colleagues to design effective treatment strategies.

• Maintain comprehensive and organized documentation, including monthly and quarterly progress notes, to comply with legal and organizational standards.

• Act as a liaison between the public, mental health resources, and community organizations, helping to bridge gaps in care and access.

• Facilitate training sessions to ensure that team members are well-informed about program plans and documentation practices, enhancing overall staff competence.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Jewish Family Care Therapy, you will work with a diverse client base that includes children, adolescents, adults, and families. Compensation and Benefits\n

The annual salary for this LPC position ranges from USD 90,000 to 100,000, depending on your experience and availability.
