Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of children in need, ensuring individualized care plans that reflect each child's unique circumstances.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs) alongside families and multidisciplinary team members.
\n
• Monitor and assess ongoing outcomes to measure the effectiveness of interventions and adapt plans as necessary to meet evolving family needs.
\n
• Provide essential support to families, equipping them with strategies and skills crucial for enhancing their child’s development within their natural environment.
\n
• Follow all Federal and State regulations to ensure compliance and maintain the highest standards of practice.
\n
• Establish a strong rapport with children, families, and team members, fostering a nurturing and engaging learning environment.
\n
• Utilize evidence-based practices effectively, incorporating the Natural Learning Environment Model into sessions designed to facilitate growth in physical, cognitive, social/emotional, and communication skills.
\n
• Offer hands-on guidance and coaching to families, helping them navigate specific challenges and reinforce developmental opportunities at home.
\n
• Collaborate seamlessly with peers and ensure open lines of communication to uphold a cohesive team dynamic and superior patient care.

Patient Population / Client Focus\n

Your work will primarily focus on children, particularly infants and toddlers facing developmental challenges. The LCSW will engage with families hailing from diverse backgrounds and will play a vital role in offering Emotional Intelligence (EI) support, significant for promoting the wellbeing of the child and their family unit. Your empathetic approach will be key in addressing each child’s unique developmental needs and the families' priorities.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.
