Work Environment\n

This role is primarily remote, allowing for flexibility in working hours and locations. However, face-to-face interaction is crucial as home visits to clients in hospice care will be necessary. The work setting emphasizes a supportive, community-oriented framework where collaborative efforts with healthcare colleagues ensure that comprehensive care is delivered effectively. Our schedule typically runs Monday through Friday, from 8:00 AM to 4:30 PM, with some evening and weekend availability depending on client needs.

Key Responsibilities\n

• Conduct detailed assessments of the psychosocial needs of seniors to understand emotional, mental, and social factors affecting their well-being.

• Develop personalized, client-centered care plans through direct collaboration with clients and their families, taking into account their individual goals and wishes.

• Provide ongoing counseling services, offering emotional support to seniors and their families as they navigate complex life transitions and challenges associated with aging.

• Advocate for clients’ needs, ensuring access to essential resources and services within the community, including healthcare and social support systems.
\n
• Collaborate intimately with an interdisciplinary care team, including nurses and healthcare professionals, to coordinate and enhance client care, sharing insights and updates regularly.

• Execute routine client visits to monitor their progress, assess living conditions, and address any safety concerns that may arise.
\n
• Facilitate effective communication among family members, working to resolve conflicts and ensuring all parties' needs and wishes are respected in the care process.

• Educate clients and families on available community resources, guiding them toward support groups and programs that foster social engagement.
\n
• Maintain up-to-date documentation within electronic health records (EHR) systems, including client assessments, care plans, and progress notes, ensuring accuracy and compliance.

• Stay informed on the latest practices, policies, and ethical standards in social work, ensuring our services remain aligned with industry standards.

Patient Population / Client Focus\n

This role focuses primarily on supporting seniors within our Hospice Center. Our patients often experience multifaceted challenges, including chronic health conditions and emotional adjustments related to aging. Understanding their unique histories and needs helps in crafting effective care strategies that enhance their dignity and overall quality of life.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Possession of a current and valid LCSW license, issued by the relevant state authority.

• A minimum of two years of professional experience in social work, ideally within a geriatric or community-centered environment.

Compensation and Benefits\n

The salary for this position ranges between USD 60,000 and USD 100,000 annually, dependent on experience and qualifications.
