Key Responsibilities\n

• Conduct thorough mental health assessments for hospice patients, taking into account their medical history, psychological state, and palliative needs.

• Develop and implement customized treatment plans that address both mental and emotional health, focusing on improving quality of life and aiding in coping strategies for both patients and families.

• Utilize evidence-based therapeutic modalities, including cognitive-behavioral therapy (CBT) and supportive therapy, to provide compassionate care.

• Facilitate individual and family therapy sessions to foster emotional expression, enhance coping mechanisms, and promote a supportive family dynamic.

• Coordinate with a team of healthcare professionals, including doctors, nurses, and chaplains, to ensure the delivery of holistic and integrated patient care.

• Respond to mental health crises promptly, utilizing effective de-escalation techniques and determining the appropriate level of care as needed.

• Offer psychoeducation to patients and their families regarding mental health conditions, available resources, and coping strategies.

• Maintain accurate and compliant patient records, treatment plans, and progress notes as per ethical guidelines and legal standards.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current and valid Licensed Clinical Social Worker (LCSW) license in Illinois.

Compensation and Benefits\n
