Key Responsibilities\n

• Conduct initial and ongoing assessments to identify the specific mental health needs of each patient, tailoring interventions accordingly.

• Maintain clear and effective communication with supervisors, direct care staff, family members, and other stakeholders regarding patient progress and treatment plans.

• Create and provide a detailed schedule for appointments, home visits, and monitoring activities to ensure organized service delivery.

• Coordinate and conduct home visits to facilitate in-home observations, chart reviews, and interviews, enabling a comprehensive understanding of clients' circumstances.

• Implement the program plan efficiently, ensuring that all services rendered are in alignment with best practices and ethical standards.

• Engage in interdisciplinary team meetings to foster collaborative treatment approaches and share insights on patient care.
\n
• Develop and maintain accurate progress notes on a monthly and quarterly basis, documenting patient advancements and program efficacy.

• Facilitate training sessions to ensure all team members are proficient and knowledgeable in implementing the prescribed therapeutic approaches and documentation practices.

• Provide follow-up support through interviews and home visits, offering continuous guidance based on individual client needs.

• Ensure comprehensive record-keeping so that relevant information is accessible to authorized personnel and stakeholders.

• Serve as a liaison between clients and the mental health community, supporting the development and implementation of therapeutic goals.

• Contribute to the creation and refinement of procedures and training materials aimed at optimizing individual care programs.

The LPC will work with a diverse client population, including both adults and youth, addressing a range of mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Current and unrestricted licensure as a Professional Counselor (LPC) within the state of Arizona.

• A minimum of two years of experience in counseling, ideally within a clinical or outpatient setting.

• Specialized knowledge or certification in family therapy, couples therapy, trauma-informed care, or substance abuse treatment is highly preferred.

Compensation and Benefits\n
