Description

• Conduct comprehensive evaluations for patients in inpatient and outpatient contexts while creating personalized treatment plans
• Deliver hands-on physical therapy interventions tailored to improve mobility, strength, balance, and functionality
• Work closely with nurses, physicians, case managers, and fellow rehabilitation experts to achieve optimal patient success
• Guide patients and their families through therapeutic objectives, discharge planning, and home exercise protocols
• Thoroughly document evaluations, treatments, and patient progress in compliance with facility and regulatory standards
• Participate actively in care team discussions and interdisciplinary meetings to ensure seamless patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory
