The Physical Therapist position at Touchstone Wellness is primarily onsite, focusing on home care settings. Key Responsibilities

As a Physical Therapist at Touchstone Wellness, your responsibilities will include:

• Conducting comprehensive assessments of patients' physical, functional, and biopsychosocial needs to design effective treatment plans tailored to their specific challenges.
• Communicating evaluation findings and treatment strategies to patients and their families, facilitating collaborative goal-setting to foster active involvement in their recovery process.
• Implementing evidence-based treatment protocols based on individual patient evaluations, adapting approaches as needed in response to their progress and challenges.
• Engaging with interdisciplinary teams, including physicians and rehabilitation specialists, to ensure a holistic approach to patient care and document critical changes in their health status that necessitate plan adjustments.
• Completing discharge planning and follow-up care meticulously, ensuring documentation is accurate and reflective of each patient's therapeutic journey, including progress notes and evaluations.
• Managing your schedule efficiently while meeting productivity expectations, and contributing to hospital and community marketing initiatives.
• Participating in department goal-setting and complying with all relevant health regulations, ensuring quality is maintained throughout our operations.
• Pursuing continuing education opportunities for professional growth while sharing knowledge to enhance team competencies.
• Promoting an exceptional patient experience, demonstrating adaptability, and engaging with initiatives that aim to remove barriers to access within the care system.

Patient Population / Client Focus

Qualifications

The following qualifications are required for the Physical Therapist position:

• A Bachelor's Degree in Physical Therapy from an accredited institution.

Compensation and Benefits
