Work Environment

This role is primarily onsite, providing services within patients' homes, where you will create a personalized, comfortable environment for rehabilitation. Key Responsibilities

• Evaluate patients' conditions through comprehensive assessments and establish individualized treatment plans based on the physicians' referrals.
• Implement a variety of treatment modalities, employing techniques such as manual therapy, therapeutic exercises, and modalities like ultrasound or electrical stimulation to enhance patient recovery.
• Educate and empower patients and their caregivers on the treatment process and home exercises designed to promote independence and healing.
• Document all patient interactions and treatment progress accurately to ensure compliance with healthcare regulations and facilitate communication with other healthcare providers.
• Set and meet utilization goals while contributing positively to the team's overall effectiveness and patient satisfaction.

Patient Population / Client Focus

You will predominantly serve a diverse patient population within the home care setting, treating individuals facing various challenges, including orthopedic injuries, neurological disorders, pediatric conditions, and sports-related injuries. Your role is crucial in helping each patient regain their independence and quality of life through compassionate, research-based physical therapy.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited APTA or CAPTE-approved program.
• A valid State License to practice physical therapy in Florida.
• Current Basic Life Support Certification from the American Heart Association.
• Strong communication skills, both written and verbal, to interact effectively with patients, families, and healthcare professionals.
• Proficient in using electronic health record (EHR) systems and Microsoft Office Suite for documentation and administrative tasks.
• A genuine passion for building relationships with patients and a commitment to their healing journey.

Preferred Experience

• Prior experience in home health physical therapy is preferred but not required.
