Key Responsibilities

• Conduct thorough physical evaluations to create individualized treatment plans based on patient needs and goals.
• Implement a variety of therapy modalities including manual therapy, therapeutic exercises, and functional training, tailored to address mobility issues, pain management, and functional limitations.
• Utilize evidence-based practices to provide gait training, balance exercises, and fall prevention strategies.
• Support patients in regaining autonomy by assisting them in performing daily activities safely and efficiently, teaching them techniques to overcome challenges.
• Educate patients, their families, and support staff regarding conditions, therapy techniques, and wellness strategies to promote long-term health.
• Document and maintain accurate patient records that align with healthcare regulations, ensuring continuity of care.
• Collaborate with nursing staff, caregivers, and fellow therapists to deliver coordinated treatment plans focused on the whole person.
• Monitor patient progress, adjusting treatment plans as necessary, and communicate changes in condition to physicians and family members promptly.

Patient Population / Client Focus

This role focuses primarily on an outpatient demographic, including geriatric patients who are navigating recovery from surgeries, injuries, or chronic conditions. You will work closely with older adults, tailoring therapy interventions that promote strength, stability, and independence. Your ability to empathize and connect with this patient population is essential to fostering recovery and improving their quality of life.

Qualifications

• Active Physical Therapist license in the state of New Jersey is required.
• Bachelor's degree in Physical Therapy; a Doctorate or Master's Degree from an accredited program is strongly preferred.
• A minimum of two years of direct patient care experience, with a preference for candidates experienced in working with geriatric or assisted living populations.
• Demonstrated clinical knowledge, adaptability to patient needs, and empathetic communication skills.
• CPR and First Aid certification is preferred, with additional certifications in geriatrics or neurological rehabilitation considered a plus.
• A valid driver's license and reliable transportation are required for travel to patients' homes if necessary.

Preferred Experience

• Experience utilizing advanced therapeutic techniques such as aquatic therapy, dry needling, or specialized exercise programs.
• Familiarity with electronic health records (EHR) systems and documentation standards in rehabilitation settings.
• A background in collaborative care models and interdisciplinary teams.

Compensation and Benefits
