Work Environment\n

This position offers a full-time remote work setting, enabling you to engage with clients and families from the comfort of your home while still making a significant difference in their lives. Regular collaboration with a multi-disciplinary team, including healthcare providers and educators, ensures seamless communication and patient-centered care. Your work hours will typically be Monday to Friday, from 8:00 AM to 4:30 PM, with occasional flexibility required for client meetings and urgent situations.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account their emotional and social environments, to facilitate effective care planning.

• Develop customized care plans in collaboration with clients and their families, focusing on their specific goals and preferences to promote holistic well-being.

• Advocate vigorously for clients’ rights, ensuring they can access necessary community resources and services that align with their care plans.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range from $60,000 to $100,000 annually, commensurate with experience and qualifications.
