Work Environment

This role is based onsite, providing care primarily in patients' homes, which allows for personalized and adaptive treatment plans tailored to individual needs. As part of our team, you will work a 40-hour week with variable weekends, ensuring flexibility in your scheduling. Key Responsibilities

• Conduct thorough evaluations of patients with functional limitations and develop individualized treatment plans based on assessed needs.
• Provide clinical consultation to healthcare providers regarding the continuity of care and collaborate with physicians on patient treatment plans.
• Document evaluations, treatment sessions, and discharge plans meticulously, adhering to all relevant laws, regulations, and professional standards.
• Implement treatment plans that incorporate evidence-based practices and continuously adjust them based on patient progress and feedback.
• Engage in community outreach activities and quality improvement initiatives, contributing to program development and the marketing efforts of the clinic.
• Supervise and mentor assigned support staff, guiding them in best practices and efficient patient handling.
• Participate in staff meetings, in-service training, and professional development opportunities to enhance your clinical skills and knowledge.

Patient Population / Client Focus

In this role, you will predominantly work with patients experiencing orthopedic issues. You will exercise your expertise in assessing and treating conditions such as post-operative rehabilitation, joint pain, and sports-related injuries. Building therapeutic relationships with patients of varying ages and backgrounds is vital. You will be called upon to apply culturally competent care strategies to meet the diverse needs of our clientele.

Qualifications

To be considered for this position, candidates must possess:
- A Doctorate in Physical Therapy from an accredited institution.
- A valid state license in Physical Therapy.
- Current Basic Life Support (BLS) certification.
- Strong verbal and written communication skills, as well as proficiency in word processing and electronic documentation systems.

Preferred Experience

While a Bachelor's Degree in Physical Therapy is the minimum educational requirement, candidates with prior experience in orthopedic therapy, particularly in home settings, will have a distinct advantage. Experience with electronic health record (EHR) systems is also beneficial.

Compensation and Benefits
