Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients, identifying emotional, social, and financial needs crucial for their overall care.

• Collaborate with multidisciplinary medical teams to develop tailored care plans that address both the physical and psychosocial aspects of patient health.

• Facilitate individual and group counseling sessions focused on issues such as grief, coping strategies, and adjustment to changing health conditions.

• Engage with community organizations post-discharge to coordinate essential services, ensuring continuity of care for each patient.

• Provide emotional support, education, and advocacy to patients and families, enhancing their understanding of available treatment options and resources.

• Participate in hospital committees, offering insights and expertise regarding social work practices and processes.

• Deliver timely crisis intervention and counseling as needed, ensuring that patients and their families are adequately supported during distressing times.

• Maintain precise documentation that complies with hospital standards and regulatory requirements, ensuring legal and ethical integrity in all processes.

Patient Population / Client Focus\n

In this role, you will primarily serve a diverse population of patients in hospice care. This includes individuals facing life-limiting illnesses and their families, who are navigating emotional challenges and complex social dynamics. Your interventions will help patients find peace and comfort while addressing their individual psychosocial needs.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Preferred Experience\n

• A minimum of two years' experience providing social work services within a healthcare or hospital setting is preferred.

Compensation and Benefits\n
