Experienced Family Nurse Practitioner for Concierge and Home-Based Care

Work Environment

This role is primarily onsite and involves traveling to various non-clinical environments, including private residences, assisted living facilities, and mobile clinics. Our FNP will engage directly with patients, coordinating closely with a supervising physician to ensure optimal health outcomes.

Key Responsibilities

• Conduct comprehensive medical assessments including taking medical histories, performing physical examinations, and evaluating risk factors for chronic disease.
• Order and interpret diagnostic tests and procedures, and develop individualized treatment plans based on findings.
• Address both chronic and acute healthcare concerns, determining medical urgencies and establishing prioritized interventions.
• Maintain accurate documentation of patient care, including medical diagnoses, treatment plans, and any referrals to specialists.
• Engage in health education for patients and their families, promoting preventive care and chronic disease management strategies.
• Work in close collaboration with a supervising physician to ensure comprehensive care and continuous support for patients when needed.
• Participate in community outreach initiatives to raise awareness of available healthcare services and preventive care programs.

Patient Population / Client Focus

The Family Nurse Practitioner will provide care across a diverse patient population, including adult and geriatric patients, children, and women’s health clients. By focusing on holistic approaches, you will address a wide range of medical needs, from managing chronic conditions like diabetes and heart disease to delivering well-child visits. Additionally, you may cater your approach to specific programs targeting particular demographics, ensuring tailored and effective healthcare solutions.

Qualifications

• Master's degree in family health or a related field from an accredited nursing program.
• Active and unrestricted national certification from a recognized body (e.g., American Academy of Nurse Practitioners or American Nurses Credentialing Center).
• Current state-issued license to practice as a Family Nurse Practitioner in Florida.
• Basic Life Support (BLS) certification for healthcare professionals.
• Valid driver’s license with a good driving record, as travel to various care locations is required.

Preferred Experience

• Previous experience in chronic disease management and preventive care is highly desirable.
• Familiarity with electronic health record (EHR) systems for patient documentation and care management.
• Experience working in home health care or concierge medicine is an advantage.

Compensation and Benefits

We offer a competitive salary range of $80,000 to $150,000 annually, commensurate with experience and qualifications.
