Work Environment

This full-time role is performed onsite within our health and wellness center in Jacksonville, Florida, with a strong emphasis on in-home patient interactions through house calls. You will benefit from a supportive work environment that values collaboration and team effort. With no requirement for on-call shifts or weekend hours, you can focus entirely on providing high-quality patient care during regular Monday, Wednesday, and Friday business hours from 8:00 a.m. to 5:00 p.m.

Key Responsibilities

• Conduct thorough health assessments and evaluations, performing onsite health risk assessments to identify and address potential health issues.
• Develop individualized intervention plans tailored to each patient’s unique health needs and goals.
• Educate patients on wellness strategies, preventive practices, and lifestyle changes conducive to better health outcomes.
• Guide patients in the management of chronic conditions, creating effective treatment plans, and ensuring continual monitoring to optimize their health.
• Coordinate and oversee the care delivered by the medical staff within the center, fostering a collaborative working relationship that enhances patient care.
• Engage actively in community wellness events and health fairs, promoting health education and fostering community engagement.
• Emphasize the importance of health outcomes over sheer patient volume, ensuring the focus remains on quality patient care.

Patient Population / Client Focus

As a Family Nurse Practitioner at Lifeline Family Care Center, you will primarily serve a diverse patient population that includes individuals from various ages and backgrounds seeking personalized and preventive healthcare. By prioritizing long-term health and wellness, you will empower patients to actively participate in their healthcare journeys, thus fostering healthier communities.

Qualifications

• A Master’s degree in Nursing with a specialization as a Family Nurse Practitioner.
• A current and unrestricted state licensure is required.
• Proficient in using Electronic Medical Records (EMR) systems, supporting efficient patient documentation and communication.
• A commitment to making a difference through quality healthcare, excellent communication, collaboration, and trust-building with patients.
• A willingness to attend community health fairs and engage in outreach programs to promote health education.

Preferred Experience

• A minimum of 2 years of experience practicing as a Family Nurse Practitioner, specifically within a primary care or family practice environment.
• Demonstrated experience in concierge medicine or similar personalized healthcare settings is preferred but not mandatory.

Compensation and Benefits

The compensation for this role ranges from $125,000 to $140,000 per year, depending on experience and qualifications.
