Key Responsibilities Conduct culturally sensitive and strength-based assessments for new patients and their families, effectively understanding their unique circumstances and needs. Provide high-quality individual and family therapy tailored to the specific needs and goals of clients. Accurately complete billing, documentation, individualized treatment plans, and insurance authorization forms in accordance with specified timelines to ensure seamless care delivery. Develop crisis plans, safety plans, relapse prevention strategies, and wellness plans, equipping clients with tools to maintain stable functioning and support their long-term mental health. Create comprehensive discharge plans that highlight ongoing care needs and community resources available after treatment. Adhere to all current federal and state laws, regulatory requirements, and Evolve and Change policies and procedures, ensuring compliance in all aspects of care. Engage in continuous learning to maintain professional competence, including attending training and pursuing continuing education opportunities. Participate actively in case consultation meetings and agency meetings to support team-based strategies for client care. Complete all required licensure and mandatory training to ensure compliance with professional standards. Patient Population / Client Focus Our staff works closely with a diverse client population, including children, adolescents, adults, and families seeking support for a variety of mental health concerns such as anxiety, depression, trauma, and relationship issues. Your role will involve tailoring therapeutic approaches to meet the specific cultural and individual needs of each client, promoting an inclusive and responsive therapeutic relationship. Qualifications A Master’s degree in social work, counseling, marriage and family therapy, or a related field that qualifies you for licensure. Current licensure as a mental health professional (LPC, LMFT, or LCSW) in the State of Georgia. Preferred Experience While the minimum qualifications are essential, candidates with additional experience in outpatient settings, familiarity with various therapeutic modalities, and proficiency in using electronic health record systems will be highly regarded. Experience with integrated care approaches and community-based resources is also beneficial.
