Work Environment

This position requires an onsite presence at a school facility in Harrison, New York. You will collaborate closely with educators, school counselors, and other health professionals to create a comprehensive health support system for students. Key Responsibilities

• Conduct thorough health assessments and wellness screenings for students, effectively identifying and addressing their health needs.
• Develop and implement individualized care plans in collaboration with students, families, and other healthcare providers.
• Educate both students and staff about health topics, including nutrition, mental health, and chronic disease management.
• Monitor and manage students’ medical conditions, ensuring that appropriate interventions are in place.
• Collaborate with multidisciplinary teams to coordinate referrals and follow-up care as necessary.
• Document patient interactions and care plans accurately in compliance with healthcare regulations and school policies.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse population of school-aged children in Harrison. Qualifications

Compensation and Benefits
