Position Overview

Empower Health NP is seeking a dedicated and compassionate Nurse Practitioner to join our dynamic team in Harrison, New York. This full-time position focuses on delivering holistic healthcare services within a school setting, emphasizing preventive care, health promotion, and the management of acute and chronic health conditions in a diverse student population. Work Environment

This role will be based onsite at a local school, creating an engaging and vibrant work environment where you can connect with students, faculty, and families. The normal work schedule spans from 9:00 a.m. to 6:00 p.m., with some flexibility for occasional evening clinics until 8:00 p.m. Participation in community outreach initiatives and health fairs during weekends may be required, ensuring that our health services reach those in need.

Key Responsibilities

• Conduct thorough health assessments and screenings for students, tailored to their specific health needs.
• Diagnose and effectively manage acute and chronic health issues, implementing preventive care strategies to promote student well-being.
• Develop and coordinate health education programs in collaboration with school staff and community organizations.
• Deliver culturally competent care while advocating for students' rights to equitable access to healthcare services.
• Ensure continuity of care by managing referrals and facilitating follow-up appointments with other healthcare providers.
• Maintain accurate and up-to-date medical records in compliance with state regulations and best practices in documentation.
• Engage in community outreach events, providing health screenings and education, thereby enhancing community health awareness.
• Stay informed on trends in public health, community health initiatives, and relevant healthcare policies impacting our patient population.

Patient Population / Client Focus

You will primarily serve school-age children and their families, addressing a range of health concerns from routine check-ups to chronic conditions. Your role is vital in promoting a healthy school environment and proactive health engagement. Furthermore, you will foster relationships with families, helping them navigate the healthcare system and ensuring they receive appropriate services.

Qualifications

• Master's degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) from recognized certifying bodies.
• A current and unrestricted Nurse Practitioner (APRN) license in New York State is required.
• Possession of a valid DEA# and NPI#.
• At least three years of clinical experience in a related field, with preference given to those with a background in primary care or community health.
• Demonstrated ability to perform robust clinical assessments and provide accurate diagnoses.
• Strong communication and interpersonal skills, enabling effective collaboration with diverse populations and stakeholders.
• Capacity to work independently as well as a collaborative team member.
• Familiarity with electronic health record (EHR) systems, ensuring efficient patient documentation and information sharing.

Preferred Experience

• Experience in an academic or public health setting.
• Proficiency in developing and delivering health education materials tailored to school-age populations.
• Prior involvement in community outreach or public health campaigns.

Compensation and Benefits
